Advanced Practitioner

The Hospital at Home (H@H) service covering Oban and Lorn has proven to be a safe alternative to hospital admission. The service provides acute care at home with the objective of preventing hospital admission or reducing length of stay in hospital.

The Advanced Practitioner within the H@H service is a key role in the assessment and management of acute and complex patients; providing a high level of complex decision making within a multidisciplinary team. It involves management of patients with acute conditions across a number of different clinical pathways, where their medical conditions can be managed safely and effectively at home.

We are looking for a WTE 1.0 advanced practitioner post for our team to provide a service across 7 days. The post will be fixed term until December 2026.
