Hospice LPN-Inpatient Nights

, a part of Empath Health is seeking a caring and motivated LPN to work with us at our Hospice House in Ocala while providing exceptional care on the following schedule:
End of the week, 7pm to 7am

• Provides ongoing assessments of physical, psychological, social and spiritual needs of the Patient/Family and initiates appropriate intervention or referral in collaboration with RN.
• Provides accurate and appropriate documentation of patient/family services as provided for in Empath Health policies and submits in a timely manner.
• Assists the RN to provide continuity of care and submits the required documentation in a timely manner. Receives orders in collaboration with the RN from physician.
• Assists with providing bereavement counseling to the family and/or primary caregiver as determined by the IDT.
• Administers medications and treatments prescribed by the attending physician in collaboration with RN.

• Minimum of 1 years’ experience preferred.
• Licensure:
• Certification:
  Valid CPR certification from the American Red Cross or American Heart Association;
  Current IV Therapy certification required or willing to obtain within six moths of hire date.
• Must be able to remain flexible and work at alternate facility where there is a business need for reassignment for one or more shifts at an alternative facility. When this occurs, the colleague is responsible for their own transportation to the alternate facility and at their expense. If mode of transportation is self-driving to that facility, the colleague must maintain an active, valid driver’s license and proof of valid automobile insurance, both of which will be placed in the colleague’s HR file.
