PositionHighlights
- Deliver comprehensive, patient-centered care for a senior-only panel
- Monday to Friday schedule, 8:00 AM to 5:00 PM with a dedicated lunch hour
- Patient volume: 10-15 per day, allowing for quality, unrushed visits
- Focus on preventive care, chronic disease management, and patient education
- Close collaboration with supervising physicians and a multidisciplinary care team
- Practice medicine in a model designed around outcomes, not volume
- Collaborative, supportive team-centered culture
- Dedicated care coordination, health coaching, and pharmacy support
- Streamlined EMR workflows for efficient documentation and reduced administrative burden

- Graduate of an accredited Nurse Practitioner program with active licensure
- Bilingual (Spanish) required to serve the local community, encouraged to still apply for other nearby positions if interested
- Confident managing chronic conditions such as diabetes, COPD, CHF, and hypertension
- Skilled in EMR documentation and committed to accuracy
- Compassionate, proactive, and patient-centered in approach
- Strong clinical judgment with the ability to think on your feet
- Excellent communication skills both with patients and team members
