Hospice LPN

Overview

Empath Health is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. Nights in patient unit.

Hours:

7p-7a full-time;
End of Week

Location:

Cates House

• Assess physical, psychological, social, and spiritual needs and coordinate interventions with the RN.
• Document services and collaborate with IDT on patient care.
• Administer medications and treatments as prescribed.
• Provide bereavement support and community education.
• Participate in team care, ensuring continuity and quality of service.
• Ensure compliance with policies, procedures, and regulations.

• Associate’s degree or equivalent experience.
• Valid Florida LPN License & Driver’s License.
• 1 year of experience preferred.
• Valid CPR and IV Therapy certification required.
• Post Offer Employment Testing (POET) or Fit for Duty examination.
• Required to work in other care centers based on business needs.
