PositionThe APRN is a clinical role that provides exceptional medical care by conducting physical exams, ordering and interpreting diagnostic tests, and developing patient treatment plans under physician supervision. The incumbent reports to the Regional Manager and collaborates with other clinical professionals to deliver high‑quality care.
Responsibilities- Practice medicine under physician supervision using sound medical judgment and ethical considerations.
- Provide care for patients with acute and chronic illnesses encountered in older adults.
- Accountable for patient care and outcomes; seek consultation from specialists when needed.
- Engage with hospitalists for inpatient patients.
- Meet all clinical obligations and compliance requirements (Accrediting Agency, CLIA, HIPAA, etc.).
- Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and complex chronic needs.
- Follow all internal policies and procedures.
- Document all patient interactions and care plans electronically within 48 hours of each visit.
- Provide patients with information necessary to promote health and prevent/manage disease.
- Interpret diagnostic data collaboratively with supervising physicians.
- Maintain patient confidentiality per HIPAA and institutional policies.
- Perform additional duties as assigned.
- Bilingual Spanish proficiency (spoken and written) required.
- Graduate of an accredited nursing program.
- Certified Nurse Practitioner with current state licensure.
- At least 1 year of professional nursing experience, preferably in primary care as a nurse practitioner.
- Experience with electronic medical records systems.
- ACLS/BLS certification (required by state).
- Strong communication, interpersonal, and presentation skills.
- Proficiency with Microsoft Office and Google Workspace.
- Full range of body motion, including handling and transferring patients.
- Stand, walk, push, bend, kneel, and reach at arm’s length and overhead for prolonged periods.
- Lift and move patients and equipment, including up to 15 lbs.
- Corrected vision and hearing to normal ranges; able to drive and have personal transportation.
