** Job Description
** We are seeking a licensed
** Physical Therapist
** to provide in-home therapy services across the South Coastal Territory. The PT will deliver care in accordance with a physician’s plan, helping patients regain mobility and independence.
** Responsibilities
* ** Evaluate patients’ functional status and develop care plans
* Assess home environments and recommend modifications
* Provide direct physical therapy treatments
* Educate patients and caregivers on therapeutic techniques and equipment
* Monitor progress and adjust treatment plans
* Collaborate with physicians and interdisciplinary teams
* Document clinical notes and progress reports
* Participate in discharge planning
* Supervise Physical Therapy Assistants per policy and regulations
** Qualifications
* ** Active California Physical Therapy License
* Minimum 2 years of physical therapy experience (home health preferred)
* Strong communication and organizational skills
* Current CPR certification
* Valid driver’s license and insured, reliable vehicle
