Sharp Health Care in Oceanside, CA is seeking a Social Service Case Manager II to join the Discharge Planning/Case Management team. You will work both independently and as part of a team to assess high-risk patients, plan safe discharges, and connect families with community resources.
Responsibilities include crisis intervention, mandated reporting, and education for patients and staff. A Master’s Degree in Social Work is required, with LCSW preferred and CCS paneled status within six months.
