Interim Health Care Hospice & Palliative Care is seeking a Hospice Registered Nurse Case Manager. The role focuses on coordinating patient care, assessing plans of care with the team, and ensuring timely communication among clinicians to adapt as patient needs change.
Qualified candidates hold an RN license, ADN/AAS (BSN preferred), CPR/FA certification, and transportation to care sites. Hospice experience is preferred; excellent communication is essential.
