Integrated Services for Behavioral Health is seeking a Care Coordinator to plan and organize a team that supports youth at home, in school and in the community to keep families together and reduce hospitalizations.
You will lead meetings, identify needs and goals with families, and ensure voice and choice in all services. Travel and trainings are required, with a strong emphasis on HIPAA confidentiality and coordination across partners.
