Occupational Therapy Assistant (OTA) – In‑House (PT or Consistent PRN)

Evolve Therapy Services, LLC is seeking a certified Occupational Therapy Assistant for our partner skilled nursing facility located in Pahokee, FL
. The role is part‑time or consistent PRN with a base pay range of $35.00/hr – $45.00/hr
.

To plan, organize, develop, and direct occupational therapy services in the facility in accordance with federal, state, and local standards, ensuring the highest quality resident care.

• Conduct regular screenings of residents to determine appropriate interventions.
• Implement and track treatment plans, following short‑ and long‑term goals.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and the interdisciplinary team.
• Develop individual therapy schedules in cooperation with other team members.
• Deliver therapy according to established plans and document/book services accurately.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards and conduct point‑of‑care documentation daily.

• Associate’s degree in Occupational Therapy.
• Active OTA licensure in the state(s) of practice.
