• Provide skilled physical therapy services in clients' homes
• Evaluate mobility, strength, and functional limitations
• Develop and implement individualized treatment plans
• Educate clients and caregivers on home exercises and mobility strategies
• Monitor progress, document outcomes, and adjust plans as needed
• Collaborate with the care team to ensure comprehensive, high-quality patient care

• Active Physical Therapist (PT) license in the state of Florida
• Degree from an accredited Physical Therapy program
• Reliable transportation and valid driver's license
• Current CPR certification
• Strong communication and time‑management skills
• Home health experience preferred
