• Provide skilled physical therapy services in patients' homes
• Evaluate mobility, strength, and functional limitations
• Develop and implement individualized treatment plans
• Educate clients and caregivers on home exercise programs and mobility techniques
  
  Monitor progress, document outcomes, and adjust treatment as needed
• Collaborate with the interdisciplinary care team to ensure quality, coordinated care

• Active Physical Therapist (PT) license in Florida
• Degree from an accredited Physical Therapy program
• Valid driver's license and reliable transportation
• Current CPR certification
• Strong communication, documentation, and time-management skills
• Home health experience preferred
