• Assess mobility, strength, balance, and functional limitations

• Develop and implement individualized treatment plans

• Educate patients and caregivers on home exercise programs and mobility strategies

• Track progress,
  document outcomes
  , and adjust care plans as needed

• Collaborate with the interdisciplinary care team to ensure comprehensive, patient‑centered care

• Active Physical Therapist (PT) license in the State of Florida

• Degree from an accredited Physical Therapy program

• Reliable transportation and a valid driver's license

• Current CPR certification

• Excellent communication and time management skills

• Home health experience (preferred)
