Care Coordinator
New Horizons of the Treasure Coast and Okeechobee is the largest mental health and addiction recovery provider in the region. We serve 15,000 children and adults annually through inpatient crisis services, a 24‑hour help line, mobile response teams, and outpatient programs across Indian River, Martin, Okeechobee, and St. Lucie counties. We also support 22,000 students in local schools and collaborate closely with courts, law enforcement, jails, and hospitals to improve community health.

- Provides time‑limited care coordination for 12‑15 identified individuals using internal utilization reviews and SEFBHN high‑utilizer recommendations.
- Coordinates services until the individual is connected to appropriate care and progress toward Care Coordination goals is achieved.
- Ensures individuals are engaged in their current setting to facilitate smooth transitions.
- Maintains an internal process to provide on‑call services 24 hours a day, seven days a week.
- Models, coaches, and supports shared decision‑making in care planning and service determination with individuals and family members, emphasizing self‑management, recovery, and wellness.
- Works with SEFBHN to develop diversion strategies to prevent individuals who can be treated in the community from entering SMHTFs.
- Master’s degree from an accredited university or college in counseling, social work, psychology, or a related human services field, plus two years full‑time or equivalent experience working with adults and children at risk.
- Professional license highly preferred.
- Valid and current Florida Driver's License with no more than 6 points in the past five years.
