Field RN - Home Health Visits. Activa Home Health (an Accord Care Company) - a licensed, Medicare-certified home health agency - is seeking experienced and compassionate Field Registered Nurses (RNs) to join our team on a part-time basis.

The Field RN is responsible for providing high-quality nursing care in accordance with each patient’s individualized Plan of Care. This role ensures that all patients receive care focused on safety, well-being, and satisfaction. Experience with Well Sky/Kinnser and prior home health experience are a plus.

• Deliver safe, effective, and compassionate care to patients following the individualized Plan of Care.

• Communicate promptly with the care team regarding changes in patient condition, ensuring effective case management.

• Provide clinical care such as:

  • Medication administration

  • Wound care

  • Infusions

  • Catheter care

  • Transferring and positioning patients safely

• Collaborate closely with the Nurse Supervisor and Manager of Clinical Practice to ensure care aligns with agency standards.

• Maintain thorough, accurate documentation in the electronic health record (EHR).

• Demonstrate empathy, professionalism, attention to detail, and a proactive approach to patient care and problem-solving.

• Current RN license in the State of Florida

• Wound care skills required

• Strong communication, teamwork, and customer service abilities

• Proficiency in using electronic health record systems ,Oasis experience for documentation and communication required

• Minimum 1 year of home health experience (preferred)

Experience

• 1 year: OASIS (required)

• Alzheimer CEU approved by DOEA & HIV/AIDS CEUS (preferred)

• 1 year: CPR card (required)

• 1 year:
  Home Health Nurse (required)
