Full‑Time RN Clinical Manager – Hospital at Home Program

Location: Stuart to Vero Beach, FL (Hybrid)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Salary: $72,000 annually + benefits

- Lead and mentor home health staff and aides
- Oversee patient care coordination and program operations
- Ensure clinical excellence and compliance with state and federal standards
- Collaborate with referral partners, physicians, and interdisciplinary teams
- Enjoy a consistent Monday–Friday schedule and supportive team environment
- Manage patient intake, authorizations, and care planning for skilled and non‑skilled services
- Conduct start‑of‑care and supervisory visits, as well as orientations and training for staff
- Provide clinical oversight aligned with best practices and agency policies
- Support and mentor field clinicians to ensure quality care delivery
- Maintain compliance with all regulatory and accreditation requirements
- Active RN license in Florida (required)
- Minimum 1 year of home care experience (preferred)
- Experience with Hospital at Home programs (highly preferred)
- Valid driver’s license and reliable transportation
- Strong communication, leadership, and time management skills
- EMR proficiency;
