Case Manager, Rehabilitation – PRN

Join Lifepoint Health® as a Licensed Case Manager in a full‑time part‑time PRN role at Mercy Rehabilitation Hospital Oklahoma City South. You will coordinate comprehensive care and discharge planning for patients in a rehabilitation setting, ensuring timely and accurate documentation and facilitating family conferences to support goal achievement.

• Coordinate comprehensive interdisciplinary care and discharge planning for rehabilitation patients.
• Develop and update the Individual Plan of Care (IPoC) in accordance with CMS guidelines.
• Collaborate with interdisciplinary teams to identify and address barriers to care.
• Maintain clear communication with patients, families, and teams; facilitate family conferences.
• Participate in performance‑improvement initiatives and maintain accurate documentation.

• Current Registered Nurse or Social Work licensure, or licensure as a Respiratory Therapist, Physical Therapist, Speech‑Language Pathologist, or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, ANCC).
• Minimum of 2 years of social work or case management experience in an inpatient setting; acute/rehabilitation hospital experience preferred.
