The Case Manager/Discharge Planner is responsible for all activities associated with the discharge planning and coordination of care and service of individual patients and/or patient populations. Promotes effective utilization of resources and assume a leadership role on the multidisciplinary team to achieve patient and physician satisfaction and optimal outcomes. Must be able to work with all disciplines to meet patient needs from admission through discharge.

Will perform duties and responsibilities in a manner consisted with the mission and values of the Oklahoma Heart Hospital.

Education: Graduate of an accredited school of nursing or social work.

Experience: Minimum of three (3) years of hospital based nursing practice or social work experience. Experience in hospital case management, discharge planning or social work a plus. Willing to pursue certification in Case Management. Proficient in Computer Knowledge.

Licensure/Certification: Current RN license in the State of Oklahoma or graduate of a social work program.
