Hospital-Community RN Liaison

This position involves accepting Hospice, Home Health, and Home Infusion referrals across various settings such as acute care, clinics, nursing homes, and rehabilitation centers.

Maintain presence in these environments, provide excellent customer service to discharge planners, social workers, physicians, and patients, facilitate timely discharges, and support continuity of care within the system.

• Registered Nurse license in applicable state required; bachelor's degree preferred.
• Minimum three years of nursing experience, with hospice and home health experience preferred.
• Strong communication, observation, assessment, and documentation skills.
• Self-directed, flexible, team-oriented, with basic computer skills and understanding of electronic medical records.
• Knowledge of current guidelines, regulatory standards, and clinical practices related to hospice and home services.
• Ability to communicate services effective to physicians and post-acute communities.
• Experience in inpatient, community, or hospice nursing is advantageous; certification in Hospice and Palliative Nursing (CHPN) a plus.
