The Case Manager, reporting to the Transitional Living Programs Manager, administers the Transitional Housing Program (TLP) at the Carolyn Williams Center (CWC) for young men aged 18‑23. The program provides 16 individual beds and supports residents toward personal and financial self‑sufficiency, permanent housing, and life‑skills development.

• Promote proactive approaches to resident, organizational, and community relations.
• Consistently support and facilitate resident participation in all program and housing requirements.
• Ensure timely notification and correspondence with all residents and volunteers.
• Address security and safety concerns of residents and volunteers.
• Secure transportation resources for residents.
• Assist in monitoring the food pantry.
• Supervise evening activities as needed.
• Maintain confidentiality and privacy of all residents.
• Ensure HIPAA compliance.
• Develop and support individual case plans for resident self‑sufficiency.
• Collect supplementary information (employment records, medical reports, etc.) to assess eligibility for community programs.
• Complete documentation, log entries, case notes, and other written information regarding resident progress and concerns accurately and timely.
• Complete requested statistics, weekly capacity reports, and HMIS data entry within established timelines.
• Consult with the Transitional Living Programs Manager and Housing Director on program and individual issues as necessary.
• Provide input for resident success during team and/or supervision meetings.

• Conduct daily property inspections of the entire facility.
• Report all maintenance work orders to the Transitional Living Programs Manager.
• Monitor residents’ weekly building upkeep chores.
• Conduct room inspections, provide residents with inspection reports, and log results in each resident file and CTrax database.
• Maintain safety, cleanliness, and overall appearance of the facility interior.
• Submit purchase orders for food and supplies to the Transitional Living Programs Manager.
• Perform other duties assigned by the Transitional Living Programs Manager and/or Housing Director.

• High School Diploma or GED required; associate degree preferred.
• Education in social work or human services is preferred.
• One year of experience in human services, homeless services, or residential programming preferred.
• Bachelor’s degree may be considered in lieu of the stated minimum experience.
• Valid driver’s license, auto insurance, and reliable transportation required for the Housing On‑Call Program.
• Clearance of background check and drug screen.
• Safety Sensitive Position under the Oklahoma Medical Marijuana Act.

• Clear verbal and written communication skills.
• Interpersonal communication, interviewing, and conflict resolution techniques.
• Ability to work independently and as part of a team comprising volunteers, professional staff, and residents.
• Patience when assisting the population served.
• Ability to handle multiple projects and tasks simultaneously.
• Non‑judgmental approach.

• Primarily indoors in a well‑lit, heated, air‑conditioned office setting.
• Moderate physical activity in an administrative role.
• Occasional high‑stress periods.
• Work schedule may vary; may require work beyond 40 hours including weekends, holidays, night shifts, and inclement weather.
