Provide manual therapy and therapeutic interventions to patients
Collaborate with interdisciplinary team members on care planning
Conduct rehabilitation exercises and interventions
Assist with discharge planning and follow-up care
Degree in Physical Therapy

Work setting can be your preference of outpatient, home health, skilled nursing
Ability to develop care plans and treatment goals
Experience in Outpatient Rehabilitation
Knowledge of skilled nursing facilities and care for older adults
