• Conduct annual health risk assessments for Medicare Advantage plan members in various locations throughout Beckham County, such as Elk City, Sayre, and Erick
• Review and evaluate patients current and prior medical history, existing conditions, and medication regimens
• Educate patients on their medical diagnoses and prescribed medications, promoting patient understanding and self-care
• Identify gaps in care and provide recommendations to address any concerns discovered during assessments
• Visits may include annual health assessments or post-discharge checks, typically lasting around 45 minutes per assessment
• No creation of treatment plans or medication prescribing is required

• Valid Nurse Practitioner license in Oklahoma
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• Current CPR certification (BLS)
• At least 6 months of recent NP experience preferred
• Excellent communication and organizational skills

Earn between $80 and $90 per evaluation, with additional mileage and administrative stipends provided.
