Primary Care Assistant – LPN (Float Pool)
Provide direct primary care support across multiple clinic settings, delivering patient-focused services and assisting clinicians during exams and procedures.
Key Responsibilities- Take and record vital signs; administer injections and medications as prescribed
- Perform and support wound care and related clinical tasks
- Assist providers with exams, procedures, and patient preparation
- Update and manage electronic health record (EHR) documentation
- Support care coordination and follow-up activities
- Educate patients on treatment plans, medications, and self-care instructions
- Adapt to varying practice needs; contribute to quality improvement and ongoing learning
- Current LPN licensure
- Strong clinical and patient-care skills with attention to safety
- Comfort with EHR documentation and care coordination
- Effective communication and patient education abilities
- Ability to thrive in collaborative, changing clinic environments
