• Conduct comprehensive assessments and create tailored support plans for children and families within the Safeguarding/Long Term team.
• Collaborate with multi-agency teams to ensure the best outcomes for children, sharing information and insights effectively.

• Undertake regular visits and meetings with children, families, and professionals to review progress and adapt care plans as needed.
