The role
Are you an experienced social worker looking to take the next step in your career within an integrated team?
We have an exciting opportunity for a Senior Practitioner to join our Integrated Intermediate Tier Services. You will work as part of the Transfer of Care Hub and the Intermediate Care Bed-Based Team, taking on supervisory responsibilities within a truly integrated health and social care system.
This is a full-time position (37 hours per week).In this role, you will:
Support safe and effective hospital discharges.
Work within a system that promotes recovery in community or home settings where appropriate.
Help coordinate a range of support options, including:
Signposting to voluntary and third sector services
Rehabilitation at home or in bed-based settings
Care packages and home adaptations
Long-term care arrangements where required

What's needed?
As a Senior Practitioner, you will:
Provide supervision, mentoring, and support to team members.
Work collaboratively with health professionals, social care colleagues, carers, families, and partner organisations.
Take a solution-focused approach, ensuring individuals’ needs are at the centre of all decisions.
Operate within the Care Act framework, supporting assessments, planning, and decision-making.
Effectively manage risk in partnership with others.
Build and maintain strong working relationships across hospital and community services.

An experienced and confident social worker
