Senior Clinical Pharmacist - Pharmacotherapy

Description

The Senior Clinical Pharmacist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients.

• Bachelor's Degree: PHARMACY from an accredited School of Pharmacy.
• Pharm
  
  D: PHARMACY from an accredited School of Pharmacy.
• Coursework/Training:
  Postgraduate Year One (PGY-1) Pharmacy Residency, or equivalent education/experience.
• Upon hire:
  Washington Pharmacist License.
• Within 2 years of hire:
  Certification from Board of Pharmacy Specialties.
• Upon hire:
  Board of Pharmacy Preceptor License (Facility Managed) – when eligible and where applicable.
• 3 years Pharmacy practice experience with at least 2 years of related experience in lieu of PGY-1 Pharmacy Residency.
