Olympia Transitional Care is a skilled nursing facility in Olympia, WA, offering long‑term care, post‑acute rehab, and geriatric outpatient therapy services. • Effectively screen and evaluate patients with cognitive and/or ADL or physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment to patients, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
• Administer various procedures as part of the rehabilitation plan.
• Assess patient progress and adjust treatments accordingly in the clinical record.
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation.
• Refer patients as needed based on outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to the physical disorders.
• Effectively communicate with the supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide a comprehensive discharge summary of services per regulatory and clinical requirements.

• Licensed Occupational Therapist.
