Position: Shared Living Provider

Description

The overall purpose of a Shared Living Provider (SLP) is to provide residential support in the SLP's home for individuals with developmental disabilities, under the supervision of the Residential Service Coordinator. Their primary responsibility is to maintain a safe and healthy home environment for the individual supported. SLPs provide basic home support and related responsibilities, including nutritional meals, transportation to appointments, day services, recreational activities, community access, cultural and religious services, and up to 24‑hour supervision as outlined in the Individual Support Plan (ISP).

The daily routine is based on the individual's choice but structured by the ISP team to help the individual learn to function independently at home and in the community.

Duties and Responsibilities
  • Implement, modify, and report on programs for individual skill acquisition according to agency policies and procedures
  • Attend to and monitor client's needs as identified in the IPS
  • Administer medication as prescribed, following doctor’s orders and agency procedures
  • Ensure clients attend medical and dental appointments
  • Assist with client financial needs, including entering transactions, balancing accounts, and budgeting income and expenses
  • Assistance with daily living needs such as grooming, hygiene, grocery purchasing, menu planning, household management, cooking, and clothing care
  • Plan and organize community and recreational activities, and arrange transportation as needed
  • Communicate with parents, guardians, supervisors, and other agency staff on client status, reporting barriers not mentioned in the IPS to the Residential Service Coordinator
  • Act as an appropriate role model and maintain a harmonious atmosphere in the residential setting
  • Communicate clearly, orally and in written form
  • Read and understand written materials and directions
  • Perform basic math functions including adding, subtracting, multiplying, and dividing
  • Possess general home management skills such as cooking, housekeeping, menu planning, shopping, and laundry
  • Know community access options and recreational opportunities
  • Have general knowledge of basic health care procedures, personal hygiene, first aid, and routine dental and medical care
  • Understand human development and needs on a general and individual level
Requirements
  • Experience preferred, but not required
  • High school diploma or GED
  • Ability to pass a State/Federal APS/CPS background check for all members in the home
  • At least 19 years of age
  • Valid driver’s license, car insurance, and current registration
  • Computer/laptop, printer, scanner, and internet for daily documentation
  • Completion of all required training
  • Detailed verbal and written communication and computer skills
  • Understanding and compliance with agency and state policies and procedures
  • Attendance at all meetings
  • Maintenance of a clean and orderly home
  • Ability to lift 50 pounds, kneel, twist, reach, crawl, and run as needed
  • Certified Home Health Aide (CHHA) preferred
  • State-approved training, CPR and first aid certification
  • Minimum of 2 years’ experience as a human services worker preferred
  • Availability to work irregular hours, including night shifts, weekends, and holidays
  • Extensive experience in supervising and assisting clients and engaging them in suitable activities
  • Experience monitoring client conditions and administering prescribed medications
  • Ability to perform basic housekeeping tasks, prepare meals, and run errands
  • Exceptional interpersonal skills, caring approach, and physical fitness
  • Ability to ensure client safety and provide comfort
