Summary Description of WorkIntensive case‑management services will be provided to all clients to coordinate and achieve health‑care goals. Decision‑making, problem‑solving, and highly developed interpersonal skills are critical.
Compensation$19.74 to $23.54 per hour, determined by total years of relevant experience.
Work Schedule37.5 hours per week, Monday through Friday, day/evening hours as needed.
Minimum Qualifications- Bachelor’s Degree in social work or related field.
- Two years’ experience in a behavioral health setting.
- Demonstrates a high level of social work skills, practice, and knowledge.
- Valid driver’s license/acceptable driving record.
- Assess holistic health‑care and social‑service needs, strengths, abilities, and desires of clients.
- Develop realistic, achievable, goal‑oriented care‑coordination plans focused on client needs and desires.
- Assist clients in addressing barriers by utilizing community resources and health‑related information.
- Coordinate services, resources, and information with clients and on behalf of clients when needed.
- Address barriers to client treatment.
- Maintain all contact documentation and billing information in the internal reporting system and provide internal reports as needed.
- Provide problem‑solving and crisis intervention to clients.
- Refer and coordinate issues that may interfere with therapy attendance, including follow‑up calls to clients who miss appointments.
- Provide transportation to clients for appointments, when necessary.
- Maintain all required reports of the position.
- Collaborate with primary care providers, behavioral health providers, and other social‑service providers to connect services, share information, and organize case conferences.
- Work within the team model, utilizing team members’ expertise when appropriate.
- Empower clients to develop skills necessary for healthy living through problem‑solving and crisis intervention services.
- Maintain required records and statistical information.
- Work in the community, client homes, attend meetings, and assist clients as needed.
- Demonstrate high energy, passion for serving people, assertiveness, and willingness to challenge normal processes to enhance client care.
- Proficient computer skills.
- Develop rapport with clients and community professional resources.
- Evaluate the efficiency and effectiveness of community service providers to ensure quality care.
- Travel locally (75%) to provide outreach services.
- Perform other program‑related duties as assigned.
- Create, maintain, and share a dynamic self‑care plan as appropriate.
- Build connections between the agency and the larger community to support fundraising and friend‑raising efforts.
- Perform essential functions on the company workspace or designated site.
