Position: Patient Care Pharmacist - Full Time - Days

Overview

Patient Care Pharmacist - Full Time - Days at Nebraska Methodist Health System, Omaha, NE. 40 hours per week with day shift and weekend rotation; may require evening rotation dependent on business needs.

Responsibilities

Essential Job Functions

  • Provide medication therapy management services in collaboration with other providers to ensure safe, appropriate, cost-effective, and efficient drug therapy.
  • Maintain competency in drug therapy for patients of all ages (neonates, pediatrics, adults, geriatrics) and across inpatient, ambulatory, and outpatient settings.
  • Participate in interdisciplinary patient care rounds, medication reconciliation, therapy monitoring, orders management, dispensing, patient education, drug information, dosing, and scholarly activities.
  • Provide both clinical and distributive tasks for safe medication use and train students and residents as needed.
  • Master Cerner pharmacy and hospital information system; proficient with technology and workflows for medication management (e.g., automated dispensing cabinets, anesthesia carts, USP IV, clinical decision support).
  • Ensure compliance with department and organization policies and with state/federal regulations.
  • Review medication orders for dose, duration, frequency, indication, allergies, interactions, and contraindications; align regimens with guidelines or evidence-based practices in absence of guidelines.
  • Adjust medication regimens per approved protocols or interdisciplinary consultation; monitor for drug interactions and outcomes.
  • Collaborate with the healthcare team in decentralized patient care areas to provide patient-centered care.
  • Participate in interdisciplinary patient care rounds and oversee participation of residents and students where applicable.
  • Perform and review medication histories and reconcile with active medication lists; coordinate changes with prescribers as needed.
  • Document medication reconciliation activities in the medical record across transitions of care (admission, transfer, discharge).
  • Complete discharge medication review and facilitate discharge process, including prescriptions and education per department procedures or on request.
  • Ensure compliance with inpatient formulary and complete medication profile reviews with consideration of patient factors, lab values, and antimicrobial therapy considerations.
  • Coordinate distribution systems, prepare and distribute medications, and communicate pending issues with team members.
  • Complete sterile, non-sterile, and hazardous drug compounding competencies as assigned.
  • Understand safety precautions including hazardous materials handling and blood-borne pathogens considerations.
  • Monitor the medication use system to ensure correct patient, drug, dose, route, time, and outcomes.
  • Serve as liaison for inpatient information for outlying needs (retail pharmacies, home health, physician clinics).
  • Identify and report adverse drug events and ensure compliance with policies; participate in committees as assigned.
  • Provide drug information and education to healthcare providers and patients; deliver in-services and lectures as assigned.
  • Precept and evaluate pharmacy residents and students during rotations; provide disease prevention programs as needed.
  • Support mission and values and complete required education/competency modules.
  • Other duties as assigned.
Qualifications

Education
Bachelor’s degree in Pharmacy or Doctor of Pharmacy (Pharm

D) required.

Experience
Two years of health care pharmacy experience preferred. Must be at least 18 years of age with no current drug or alcohol-related convictions; other requirements as stated.

License/Certifications
Current active Pharmacy Licensure for work state;
American Heart Association BLS preferred; ACLS preferred; completion of ASHP-accredited residency or certifying program preferred.
