Shared Provider

Responsibilities

• Provide residential support to individuals from the SLP’s own home, including documentation of services.
• Administer medication and supervise clients.
• Teach newly acquired skills and promote greater independence.
• Ensure all work complies with Cornerstone policies and state and federal regulations.
• Maintain timely completion of documentation to receive monthly bonuses.

SLPs act as subcontractors, making their own schedules and ensuring compliance with all relevant policies. Cornerstone offers advice and assistance but does not direct how work is performed.

Compensation is based on the level of care provided and the provider’s experience. Monthly bonuses reward timely completion of documentation. Positions are highly tax-advantaged;
Medicaid waiver services performed in a provider’s own home do not require reporting earnings on IRS income.

• Must pass DHHS background checks for all persons that will be living in the SLP’s home.
• Must not have a felony conviction.
