Ambulatory Care Pharmacist - Heme/BMT
Join Nebraska Medicine as an Ambulatory Care Pharmacist supporting patients with hematologic malignancies and bone marrow transplant needs. This position is located in Werner Buffet Cancer Center and provides longitudinal, outpatient medication management across all phases of care, including pre‑treatment, active therapy, transplant, and long‑term follow‑up.
Shift- First Shift (M‑F 8:00‑4:30 p.m. or 8:30‑5:00 p.m.)
- Full‑Time (40 hours/week), Benefits Eligible
- Deliver patient‑oriented pharmaceutical care services, including creation and maintenance of pharmaceutical care plans.
- Perform cost‑effective therapeutic drug monitoring, drug utilization evaluation, and guideline development.
- Disseminate new knowledge, utilization trends, and outcome information related to medication use and pharmaceutical care programs.
- Lead education of other disciplines regarding appropriate use of drug therapy.
- Collaborate within multidisciplinary care teams to optimize complex medication regimens, monitor treatment response and toxicity, and support safe transitions between inpatient and outpatient care.
- Educate patients and staff to enhance adherence and evidence‑based decision‑making in a high‑acuity ambulatory setting.
- Graduate of an ACPE‑accredited Doctor of Pharmacy program, or hold a Bachelor or Master of Pharmacy degree with 10 years of experience.
- Graduate of an ASHP‑accredited PGY1 or PGY2 residency relevant to the practice area, or possess 4 years of pharmacist experience in the relevant area of practice, or have pharmacist experience within a Nebraska Medicine ambulatory care clinic.
- Demonstrate excellence in clinical practice and the ability to consistently meet and contribute to updating standards of practice.
- Show maturity and leadership skills.
- Licensed in the state of Nebraska or in the jurisdiction where pharmacy practice is conducted upon start or transfer.
- Basic Life Support (BLS) certification from an American Heart Association (AHA) endorsed program, effective for new hires starting January 1, 2023 or later.
- Board of Pharmaceutical Specialties (BPS) certification relevant to practice.
