Position OverviewThe Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
Essential Job Functions- Delivery and coordination of quality patient care in compliance with physician orders.
- Continuous observation and assessment of patient condition and care needs, reporting changes to supervisor/physician as appropriate.
- Timely documentation of all activities, assessments, nursing actions, responses and care coordination.
- Participation and implementation of the nursing care plan.
- Initiation of appropriate nursing actions based on assessment to achieve expected outcomes.
- Intervention and care to prevent or reduce risk in response to changes in patient needs.
- Compliance with Aveanna policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
- Use of infection control measures to protect staff and patients according to OSHA standards.
- Patient and family education regarding disease process, self‑care techniques and prevention strategies.
- Participation in educational programs and continuing education to maintain nursing competencies.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification (hands‑on).
- Demonstrated proficiency in clinical assessments, documentation and compliance with nursing care and policies and procedures.
- Must have reliable transportation.
- Must be able to speak, read, write and understand English.
- Must be able to lift 50 pounds and reposition patients without assistance.
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Experience working under a nursing license in California, South Carolina or Louisiana as specified.
- Speak, read, write and understand English.
- Travel and relocate in the field as required.
- Lift up to 50 pounds and reposition patients independently.
- Prolonged walking, standing, bending, kneeling, reaching and twisting.
- Sufficient visual and hearing acuity.
- Ability to respond physically and mentally to emergency situations.
- Ability to work in varied environments, possibly exposed to allergens, bodily fluids or infectious diseases.
