Trafficking Case Manager III - Heartland Housing Sanctuary (Part Time, Overnight)

Summary Description Of Work

This position will work as part of the human trafficking team, collaborating closely with several social service organizations to ensure survivors can access program services with minimal barriers. The trafficking case manager will work in the crisis stabilization shelter and provide comprehensive case management, safety planning, and ongoing support to survivors of human trafficking, specifically those who have been sexually exploited and/or trafficked.

Compensation

Between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience).

Work Schedule

28 hours per week:
Sunday evening from 9:30 pm-7:30 am;
Monday and Tuesday from 10:30 pm-7:30 am.

Minimum Qualifications
  • Bachelor’s degree in social work or human service field, and two years direct care experience in Human Services preferred; equivalent employment experience may be considered.
  • Valid Driver’s License with an acceptable driving record.
Essential Duties And Responsibilities
  • Provides crisis intervention and support to survivors of trafficking.
  • Demonstrates advanced crisis intervention skills while working with a diverse clientele.
  • Assists with meeting basic needs at a crisis stabilization shelter, including developing and revising safety plans and connecting clients to medical, therapeutic, or other services identified the intake and assessment process.
  • Accurately and thoroughly completes all required paperwork and data entry per agency policies and funding guidelines within prescribed timelines.
  • Provides client transportation in personal and agency vehicles.
  • Participates in monthly team meetings with HFS program staff.
  • Follow program policies and procedures, including agency protocols related to medication administration.
  • Completes all required monthly, quarterly, and annual reports for quality improvement and grant purposes.
  • Creates, maintains, and shares a dynamic self‑care plan as appropriate.
  • Strives to make connections between the agency and the larger community to contribute to fundraising and outreach efforts.
  • Perform essential functions of the job at the company’s physical work site.
  • Travel in town is required, including use of personal vehicle.
  • Works independently with little supervision and as part of a team.
  • Establishes regularity and dependability in scheduling appointments and in reporting accuracy.
