Family Violence Prevention Coach
The Family Violence Prevention Coach provides trauma-informed, culturally responsive, survivor-centered, and family-centered support, education, advocacy, service coordination, and resource navigation to individuals and families impacted by domestic violence, intimate partner violence, and family violence. Through evidence-based and strengths-based practices, the Coach supports participants in identifying goals, strengthening life and parenting skills, increasing safety and stability, and connecting with community resources. The position maintains accurate documentation, supports program evaluation and grant compliance, participates in outreach and recruitment, and collaborates with community partners to strengthen participant outcomes.
Duties& Essential Job Functions
- Provide trauma-informed, culturally responsive, survivor-centered, and family-centered coaching, education, support, advocacy, and skill-building services to participants and families.
- Maintain an assigned caseload and provide home-, school-, office-, and/or community-based services as appropriate and in accordance with agency expectations.
- Complete screenings, assessments, individualized service plans, safety plans, progress reviews, and other required participant documentation.
- Provide parenting education, life-skills coaching, safety planning, crisis support, and solution-focused interventions using evidence-based and strengths-based practices.
- Assess barriers impacting participant success and assist participants in identifying goals, building skills, and developing strategies to achieve desired outcomes.
- Provide resource navigation and referrals for housing, behavioral health, legal services, employment, childcare, transportation, financial assistance, and other social-service needs.
- Coordinate referrals, follow-up services, and ongoing participant engagement to support successful outcomes and continuity of care.
- Assist participants in developing healthy informal support systems and strengthening connections with family, kinship, and community resources.
- Facilitate educational workshops, support groups, listening circles, and other program activities as assigned.
- Maintain timely and accurate electronic and paper documentation, including progress notes, assessments, service plans, attendance records, referrals, and outcome measures.
- Utilize agency electronic documentation, data-tracking, and reporting systems to maintain accurate participant and program records.
- Collect demographic, service-delivery, output, outcome, and performance-measure data to support program evaluation and continuous quality improvement.
- Ensure program activities and participant documentation comply with agency policies, grant requirements, funding expectations, confidentiality standards, and applicable program guidelines.
- Assist with grant and program reporting by providing accurate and timely participant, service, and outcome information as requested.
- Participate in participant recruitment, community outreach, presentations, events, and engagement and retention efforts.
- Develop and maintain effective working relationships with participants, CHD staff, schools, healthcare providers, behavioral health agencies, victim-service providers, faith communities, and other community partners.
- Participate in supervision, staff meetings, case consultations, program planning, professional development, and quality-improvement activities.
- Protect participant rights, dignity, confidentiality, safety, and self-determination while maintaining appropriate professional boundaries.
- Implement CHD's mission, vision, values, program goals, policies, and procedures and demonstrate professionalism through role modeling and respect.
- Perform other duties as assigned to support CHD's prevention, education, and family-support programs.
- Bachelor's degree in social work, public health, counseling, psychology, human services, behavioral health, criminal justice, or a closely related field preferred. An equivalent combination of relevant education and experience may be considered.
- At least one (1) year of direct-service experience in social services, human services, behavioral health, victim services, family support, or a related field preferred.
- Experience providing trauma-informed care, crisis intervention, survivor advocacy, case management, resource navigation, or family support preferred.
