Description

The Foster Care Caseworker will work closely with the Foster Care Department, contracting County Department of Social Services, and family members to successfully carry out the Foster Care Caseworker portion of contract requirements. The Foster Care Caseworker particularly is responsible to provide Foster Care Case Management and (as defined by contract) may be required to assess client and family needs and complete or participate in a visitation plan, safety plan, treatment plan and discharge plan.

Will be required to evaluate, coordinate, and provide necessary referrals for services and/or treatment as described. They will also provide in-home supportive casework counseling services to clients, complete required assessments, and assist individuals and families by helping to articulate goals and providing needed information. This person works closely in partnership with the families, foster families, County workers and other community partners.

  • Assist and support foster youth while in our program with any services to ensure their safety, well-being, educational, medical/mental health, and family needs are taken care of.
  • Provides services and support to the birth families of the youth in the program to assist with the permanency goal.
  • Assures that job‑related activities are following Glove House policies and procedures, OCFS, State and Federal regulations, and relevant professional association, ethical standards, accreditation standards, and the law.
  • Perform Foster Care Case Management as defined by the County Contracts. This includes treatment planning, monthly contact with schools/therapists/county CW, engaging biological families, permanency planning, independent living skills, family and sibling visitation and face‑to‑face contacts.
  • Provide or arrange transportation of youth to appointments and visitation as needed.
  • Participate as a team member of the Foster Care Department and may be asked to help facilitate recruitment efforts, and area presentations.
  • Provides support and feedback to discharge resources surrounding their strengths and needs to have their children returned to them. Including monitoring of their required court order stipulations and initiating discussions surrounding barriers of permanency.
  • Completes monthly home‑visits of discharge resources
  • Provide supportive casework counseling services for foster parents, including education and crisis management.
  • Provide case management services including referrals, connections, and links where appropriate.
  • Train direct care team members regarding components of the foster care management contract.
  • Participate in foster youth treatment team meetings.
  • Generate and maintain necessary reports and paperwork (i.e., Quality Assurance and program reports).
  • Assures all documentation of contacts and situations are completed in a timely fashion in my Evolve and CONNECTIONS
  • Utilize the agency EMR, my Evolv, in entirety to accurately represent the youth’s record
Schedule Expectations

The incumbent will work an agreed upon standard scheduled week, which may include weekends, holidays, or evenings. The ability to work flexible hours is required, as is the ability to provide on‑call support when scheduled or to meet business needs. Travel will be as needed, to conduct Glove House business‑related activities. Generally, duties of this position require at least 80 % time at work/face‑to‑face time with clients, co‑workers, and supervisor.

Up to 20 % may be approved to work from home as approved by supervisor.

Requirements

All degrees must be from an accredited university.

  • Bachelor’s degree required
  • Minimum 3+ years’ experience working with children and families in residential, group, or counseling child welfare capacity
  • Must have a valid driver’s license
