Job Description
Support the delivery of Aberdeenshire Health and Social Care Partnership Reablement and Responder Service by providing a point of contact for service users, families, staff and other professionals to schedule and deploy carers in the most efficient and effective manner to provide planned and unplanned care to service users.
There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the Service and the organisation

Responsibilities• Act as “first point of contact” for the Reablement and Responder Service, taking ownership of requests to ensure an appropriate response is provided and queries are resolved at the point of contact, or escalated to an appropriate person as necessary.
• Use a scheduling and monitoring system to Input service user support plan information.
• Use a scheduling and monitoring system to develop and maintain work schedules for care staff based on service user support plans and staff availability.
• Update schedules in response to staff absences, emergencies, or changes in service user needs, ensure all visits are covered and that continuity of care is maintained where possible and communicate visit times and changes to service users and their families.
• Provide support and advice to frontline staff in line with service policy and standard operating procedures
• Maintain and record accurate records safely within the required systems and provide information when required.
• Report on missed visits, late arrivals, and other service delivery issues to the Reablement and Response Coordinator.
Sound knowledge of geography of central Aberdeenshire.
IT literate in Word, Excel, email and Microsoft packages
Ability to work Monday – Friday
Hybrid/Home working
Ability to travel to team meetings
