Meon Health Practice - Salaried GP - Permanent (min 4 sessions)
We are looking for an enthusiastic,dynamic GP to add to our brilliant and diverse primary care team of socialprescribers, pharmacists, physiotherapists, nurses and healthcare assistants,advanced nurse practitioners, physician associates, care coordinators, salaried
GPs, retainer GPs and GP partners.
Meon Health Practice is an innovative and high achieving, but more importantly, friendly and welcoming practice on the western edge of Fareham. We have three sites which sit at the centreof their respective communities, while immediately around us the South Downs (forland-based GPs) or the Solent (for water-based GPs) are outstandingly beautiful.
We offer a flexible and modern general practice working day, which might include remote working or staggered start and finish times. Youcould provide traditional primary care or spend time with our integrated teamsin urgent care, complex patients or those patients in residential care. The salary will be competitive and dependent upon your experience, and includes indemnity, sixweeks of annual leave and a week of study leave.
Main duties of the job- The post-holder will make themselves available to undertake a variety of duties, including surgery consultations, triaging telephone consultations, queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- Developing care plans for health in consultation with patients and in line with current practice disease management protocols
- Providing counselling and health education
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
- Compiling and issuing computer-generated acute and repeat prescriptions
- Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
- In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.

Meon Health Practice is made up of 3 sites - The Highlands Practice, Jubilee and Whiteley Surgery. Our staff can work across any of the sites listed.

Job summary:
