Family Physician – Home-Based Primary Care

• Provide longitudinal primary care to seniors in their homes, retirement homes, and assisted living facilities.
• Conduct regular home visits and manage chronic conditions, palliative care, and preventive services.
• Collaborate with an interdisciplinary team (physicians, nurses, physiotherapists, rehabilitation assistant) for holistic care.
• Maintain documentation and billing through the MSP Longitudinal Family Practice (LFP) model.
• Advise patients and family caregivers on health management to improve quality of life and prevent hospitalization.
• Participate in team meetings and continuing education opportunities.

• Licensed to practice as a physician in British Columbia.
• Current residence in the Greater Victoria Area.
• Experience in home-based or community primary care preferred.
• Proficiency using the Oscar electronic medical record system is an asset.
• Strong communication skills and commitment to patient-centered care.

• Daytime clinical hours only – no evening, weekend, or statutory holiday work.
• No on-call shifts.
• Flexible appointment scheduling with patients told AM or PM.
• Locum coverage provided for absences.
• No administrative or management duties assigned.
• Continuing education support.
• MSP billing via LFP model with average full-time earnings of $350,000 per year.

The practice is fully community-based and does not involve long-term care or nursing home work. Patients range from independent to extreme frailty, and care is delivered in their own homes, assisted living facilities, or retirement homes.
