Care home medicine brings together some of the most rewarding parts of general practice: knowing people  over time, making sense of complexity, supporting families and helping patients receive care that reflects  what matters to them. At the One Weston Care Home Hub, GPs have the time, continuity and  multidisciplinary support to do this work well.
** THE OPPORTUNITY
** We are looking for an experienced, compassionate GP to join our established Care Home Hub, supporting approximately  450 care home beds across Weston-super-Mare. This is complex, relationship-based general practice, not a narrow or  routine care home role.

You will hold an allocated caseload, allowing you to build continuity with residents, families and care home teams. You will  also work alongside GPs, advanced clinical practitioners, senior nurses, pharmacists and colleagues in palliative care and  mental health. Clinical decisions are shared, different perspectives are valued and complexity is not carried alone.
** WHAT MAKES

THE ROLE DIFFERENT*
* • Meaningful clinical work. Time to understand the whole picture and make considered decisions, rather than repeatedly  addressing isolated problems.  
• A true MDT. Regular team check-ins, shared clinical thinking and specialist MDT support for palliative care, dementia, mental  health and complex care.  
• Continuity and a manageable workload. An allocated caseload, excellent administrative support and no ordinary practice  workload or duty-doctor commitments.  
• Time to support others. Dedicated space to provide clinical supervision, advice and feedback to ACPs and senior nurses  within the MDT. This is a fundamental part of the GP role, and the two-way learning is one of the most fulfilling parts of the job.
** THE CLINICAL WORK
** The role combines proactive and responsive care, with particular emphasis on advance care planning, palliative care,  multimorbidity and diagnostic uncertainty. You will undertake holistic assessment, support treatment-escalation and  ReSPECT planning, review residents whose health is changing, and help patients and families navigate difficult decisions  with clarity and compassion.

The Hub also offers supported opportunities to build confidence in safeguarding, mental capacity and best-interest  decision-making, learning disability, and the care of deteriorating people living with frailty. These are increasingly important  capabilities for the future of general practice and neighbourhood health.

THE ROLE WILL SUIT
** You will be a confident generalist who values continuity, careful clinical reasoning and collaborative working. You should  be able to provide clinical supervision to ACPs and senior nurses and be comfortable holding clinical responsibility while  drawing appropriately on the team around you. Experience or a developing interest in palliative care, frailty or  multimorbidity would be particularly welcome.

You do not need to arrive as a service-improvement expert or established clinical leader. For those who want it, there is  scope to develop these capabilities through pathway improvement, teaching, quality improvement and the continued  development of neighbourhood-based complex care.

* ** Salary:** £11,400 per session per annum  
** Pattern and hours:
** Four sessions / two days per week preferred; other patterns considered. Weekday, in-hours  service with no evening or weekend commitments

** One Weston Care Home Hub at 168 Medical, with work in surrounding care homes and some  scope for remote working
