As a Geriatrician (Home-Visiting Physician), you will provide direct medical care to elderly patients in their homes. You will diagnose and manage chronic conditions, collaborate with interdisciplinary teams, and guide families on complex healthcare decisions, always prioritizing patient well-being and dignity.

• Conduct in-depth medical evaluations and develop personalized care plans.
• Manage chronic conditions, adjust medications, and address acute concerns.
• Collaborate with NPs, PAs, nurses, therapists, and social workers.
• Educate patients and caregivers on disease prevention and health maintenance.
• Document patient encounters and maintain compliance with regulatory standards.

• Board-certified physician with specialization in geriatrics.
• Licensed to practice medicine in California.
• Experience in home-based care models and geriatric syndromes.
• Excellent communication, empathy, and problem-solving abilities.

You'll lead medical care for elderly patients in their homes - performing complex assessments, managing multiple chronic conditions, coordinating with specialists, and guiding families through difficult care decisions. This is medicine as it should be: longitudinal, relational, and centered on what matters most to each patient.

Education:

MD or DO degree, current state medical license, geriatric fellowship preferred

Experience:

Board certification in geriatrics + 5+ years clinical experience preferred
