Diabetes and You Clinical Programme Coordinator

Location: Hybrid - Home working, community venues, GP Practices and remote delivery

Hours: 15 hours per week

Contract: 12 month fixed term

The Diabetes and You Clinical Programme Coordinator is a clinically informed education and programme delivery role supporting the operational management, development and delivery of the Diabetes and You structured education programme across Cornwall and the Isles of Scilly.

Main duties of the job

The post-holder will provide clinical insight and operational leadership to support safe, effective and sustainable delivery of Diabetes and You aligned to NICE structured education standards and QISMET requirements. The role will support delivery of:

  • patient structured education courses,
  • educator forums and mentorship,
  • Train the Trainer activity,
  • programme reporting and evaluation,
  • ongoing development of educational content and resources.

The post-holder will work closely with the Education Lead, educators, Primary Care Networks (PCNs), practices and wider system partners to improve access, quality and consistency of structured diabetes education across Cornwall and the Isles of Scilly.

Job responsibilities Programme Coordination & Operational Delivery
  • Support day-to-day operational management of the Diabetes and You programme.
  • Coordinate local delivery of Diabetes and You courses across Cornwall and the Isles of Scilly through PCNs, practices and commissioned educators.
  • Plan and coordinate educator delivery, venues and programme activity.
  • Maintain oversight of programme activity, registrations, attendance and completion data with administrative support.
  • Support programme reporting, evaluation and quality monitoring requirements.
  • Develop and implement innovative operational processes to improve delivery and accessibility.
  • Collaborate with the Education Lead and Training Hub team to support sustainability and continuous improvement.
  • Communicate and engage with PCNs, practices and system partners regarding the programme.
  • Identify gaps in delivery capacity and develop solutions to maintain equitable access across Cornwall and the Isles of Scilly.
  • Explore and develop new approaches to improve engagement, accessibility and uptake of structured education.
Structured Patient Education Delivery
  • Deliver Diabetes and You structured education sessions to patients, carers and families where appropriate.
  • Support and facilitate delivery capacity across PCNs and educator teams, providing direct delivery when required to aid continuity, workforce development or gaps.
  • Facilitate group education sessions in an engaging, inclusive and supportive manner.
  • Promote patient self‑management, behaviour change and understanding of type 2 diabetes.
  • Deliver both face‑to‑face and virtual education sessions as required.
  • Ensure delivery aligns with the agreed curriculum, programme standards and educational principles.
  • Support engagement and re‑engagement of participants when needed.
  • Maintain sensitivity to emotional, behavioural and educational needs of participants.
Educator Support & Workforce Development
  • Recruit, onboard and provide ongoing support to Diabetes and You educators.
  • Deliver Train the Trainer sessions and educator development activity.
  • Support educator forums, Communities of Practice and peer support activity.
  • Mentor, guide and support educators delivering the programme.
  • Facilitate workforce development aligned to structured education delivery and long‑term conditions management.
  • Contribute to ongoing review, innovation and development of educational materials, presentations and programme resources.
Clinical & Quality Support
  • Provide clinical insight to support safe and evidence‑based programme delivery.
  • Support curriculum updates to ensure content reflects current evidence, guidance and best practice.
  • Support quality assurance processes aligned to QISMET and NICE structured education standards.
  • Contribute to audit, evaluation and continuous improvement activity.
  • Escalate risks, issues or governance concerns appropriately.
Collaboration & System Working
  • Develop effective relationships with PCNs, GP practices, educators and wider system partners.
