In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment.
Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Other responsibilities include:
- Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
- Help patients with mobility, movement, and regain their independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure a quality level of care.
- Evaluate and report patients’ progress to the MD.
- Provide skilled services in the home setting to evaluate and address patient needs.
- Collaborate with a multidisciplinary team to support patient goals and safety.

- Current unrestricted CA Physical Therapist License
- Current BLS/CPR Certification
- Valid CA Driver's License
- Knowledge of local community resources
- Knowledge of development, behavior, and cultural considerations in healthcare
- Knowledge of state and federal homecare regulatory guidelines (preferred)
- Home health care experience (highly preferred)
- Experience with high-acuity patients (a plus)
- Experience with interdisciplinary teams (highly desired)
- Case Management fundamentals (highly desired)
- Self-directed, empathetic, critical thinker with outstanding interpersonal skills
- Leadership and organizational abilities

Wage range:
Physical Therapist, Level 1: $120,000 - $135,000 per year, plus up to $10,000 annual bonus (paid quarterly). Starting pay is commensurate with relevant experience. We offer generous bonus plans, comprehensive health benefits, and 401k (up to 4% match). As a practice, most PT Case Managers are hired as PTIs and may be promoted to PTII with higher salary and bonus potential.
