JOB DESCRIPTION
POSITION TITLE:
Nurse Wellness Educator
POSITION SUMMARY:
Under the supervision of the Chief Medical Officer, plans, develops, and implements diabetes education programs for both individual patients and groups. Develops and implements protocols for administering Medicare Initial Preventive Physical Exams (IPPEs) and Annual Wellness Visits (AWVs). Collaborates with Patient-Centered Medical Home (PCMH) committee to prepare annual submission to National Committee for Quality Assurance (NCQA) for re-certification.

PRIMARY RESPONSIBILITIES:
Location:
Attendance is required during regular business hours at assigned CHCFC site(s). Remote work may be arranged by approval of CMO, when appropriate.
Other:
Bilingual/bicultural strongly preferred; commitment to CHCFC’s mission and ability to work with diverse medically underserved population required.
REQUIRED SKILLS,
EDUCATION & EXPERIENCE:
Education:
Certified Diabetes Educator/CDE and Registered Nurse (RN) or Registered Dietician required. Bachelor’s degree preferred for RN and masters preferred for RD..
Experience:
Two or more years in public health or office nutritional counseling strongly preferred.
Skills:
Strong knowledge of dietary counseling regarding chronic disease factors (cardiovascular, hypertension, diabetes) and obesity required. Strong teamwork and community activism skills.

The work requires some travel to and from evening clinics and training sessions as well as the ability to adjust work schedule to accommodate periods of extreme activity.
WORKING CONDITIONS:
While performing the required duties of this job, the employee is exposed to weather conditions prevalent when traveling between Health Center locations where medical care is provided. This position performs tasks that involve exposure to blood, body fluids or tissues.
