Nonotuck provides personalized supports to people by matching them with caring providers who open their hearts, their minds and their homes.

Just shy of 1,200 people receive Caregiving with Love in either Shared Living or Adult Family Care. Position Overview

Integration Specialist (Full-time Non-Exempt, Monday-Friday 30 hours/week)

Nonotuck Resource Associates is actively seeking a dedicated Integration Specialist to join our team and provide community and home services, along with direct care supports to individuals with developmental disabilities enrolled in our Community-Based Day Supports program. If you are passionate about fostering independence, creating meaningful connections and supporting individuals in their journey towards a full community-based life, we invite you to apply for this rewarding position.

As an Integration Specialist, you will have the primary responsibility for at least one person supported, with opportunities for cross‑training with other individuals. Your role encompasses a variety of duties aimed at promoting independence, fostering relationships, and ensuring a positive community-based experience.

This is a unique position in which hours would be split between our Brooklyn St. Project in North Adams and our Community-Based Day Services in Pittsfield, Lee and Adams.

Key Responsibilities

Individualized Support

  • Develop a deep understanding of each person supported, including their unique strengths, needs, goals, and desires.
  • Assist individuals in articulating their needs, goals, and desires to facilitate personalized care.

Team Collaboration

  • Work collaboratively as a member of a team to support individuals in developing a full community-based daytime life.
  • Collaborate with Program Director, Assistant Program Directors, Shared Living Providers, and other stakeholders.

Positive Image and Relationships

  • Support and exert personal initiative to ensure that each person presents a positive image of themselves and the program.
  • Assist individuals in developing long‑lasting and meaningful relationships with family, friends, community members, co‑workers (where applicable), and becoming active community participants.

Community Engagement

  • Assist individuals in participating fully in community activities, including group memberships, volunteer commitments, educational opportunities, leisure and social pursuits, and employment where applicable.

Documentation and Monitoring

  • Complete Individual Service Plan goals and activities in collaboration with the team.
  • Monitor activities of daily living (ADL) and provide necessary support.

Communication

  • Work closely with guardians, family members, community health care practitioners, and other institutions to ensure comprehensive and coordinated care.
Qualifications
  • 2 years experience working with individuals with developmental disabilities preferred.
  • High School Diploma
  • Computer and organizational planning.
  • Strong interpersonal and communication skills.
  • Ability to work collaboratively in a team environment.
  • Flexibility and adaptability to meet the changing needs of the individuals supported.
  • Community experience that expands specifically to the Department of Developmental Services, Adult Foster Care Services, HCBS waivers (supporting those with a Brain Injury or those who have chronic conditions affecting their availability to live safely in the community)…
