• Facilitate completion of assessments in nephrology offices, dialysis clinics, or via telehealth
• Perform outbound calls for scheduling and reminders, and welcome new patients to the program
• Maintain accurate documentation and communicate patient updates to the care team
• Coordinate care by ensuring access to eligible resources and assisting with referrals
• Complete non‑clinical screenings and assist the clinical team as directed

• High School Diploma or GED required;
  Associate’s or Bachelor’s Degree preferred
• 2+ years of professional experience in customer service or telephonic outreach roles preferred
• Prior healthcare experience preferred
• Intermediate proficiency in Microsoft Word, Excel, PowerPoint, and Outlook
• Strong verbal and written communication skills and ability to manage time effectively

• Experience in patient‑facing roles or telephonic outreach within healthcare settings
• Demonstrated ability to build trust and engage patients effectively
• Knowledge of healthcare resources and programs available for patients
