This role is work‑at‑home with 25‑50% travel within Somerset County, NJ (50‑mile radius from applicant’s home).

Standard business hours Monday‑Friday 8:00am‑5:00pm EST.

The ICM Case Manager develops a proactive plan of care to address identified issues and enhance short‑ and long‑term outcomes, supporting overall member wellness.

Join our team serving dual‑eligible populations (Medicare & Medicaid) with complex health and social challenges. The ICM Care Managers are frontline advocates, assessing, planning, and coordinating all case‑management activities to improve member health.

• Use clinical tools and data review to evaluate member needs and benefits.
• Apply clinical judgment to design strategies that reduce risk factors and address complex health and social indicators.
• Conduct assessments using multiple sources (e.g., claims) for all conditions, including comorbidities.
• Assess need for referrals to clinical resources and interdisciplinary team members.
• Collaborate with supervisors and stakeholders to overcome barriers and present cases at interdisciplinary case conferences.
• Follow case‑management processes per regulatory and company policies, utilizing motivational interviewing to engage members.

• Remote role with 25‑50% travel; requires a dedicated, interruption‑free workspace.
• Dependents must have separate care arrangements; continuous care responsibilities during shift times are not permitted.

• Minimum 3+ years of clinical practice experience.
• Active and unrestricted RN licensure in NJ.
• Willing to travel 25‑50% using own vehicle; reliable transportation required.
• Must reside close to or within Somerset County, NJ.

• Certified Case Manager.
• Minimum 2+ years of Care Management, Discharge Planning, or Home Health Care Coordination experience.
• Independent, virtual collaboration skills.
• Analytical, problem‑solving, communication, organizational, and interpersonal skills.
• Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications.
• Bilingual preferred.

• Associate’s Degree required.
• Bachelor’s Degree preferred.
