Position SummaryThe ICM Case Manager develops a proactive plan of care to address identified issues and enhance short‑ and long‑term outcomes, supporting overall member wellness.
Our MissionJoin our team serving dual‑eligible populations (Medicare & Medicaid) with complex health and social challenges. The ICM Care Managers are frontline advocates, assessing, planning, and coordinating all case‑management activities to improve member health.
Key Responsibilities- Use clinical tools and data review to evaluate member needs and benefits.
- Apply clinical judgment to design strategies that reduce risk factors and address complex health and social indicators.
- Conduct assessments using multiple sources (e.g., claims) for all conditions, including comorbidities.
- Assess need for referrals to clinical resources and interdisciplinary team members.
- Collaborate with supervisors and stakeholders to overcome barriers and present cases at interdisciplinary case conferences.
- Follow case‑management processes per regulatory and company policies, utilizing motivational interviewing to engage members.
- Remote role with 25‑50% travel; requires a dedicated, interruption‑free workspace.
- Dependents must have separate care arrangements; continuous care responsibilities during shift times are not permitted.
- Minimum 3+ years of clinical practice experience.
- Active and unrestricted RN licensure in NJ.
- Willing to travel 25‑50% using own vehicle; reliable transportation required.
- Must reside close to or within Somerset County, NJ.
- Certified Case Manager.
- Minimum 2+ years of Care Management, Discharge Planning, or Home Health Care Coordination experience.
- Independent, virtual collaboration skills.
- Analytical, problem‑solving, communication, organizational, and interpersonal skills.
- Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications.
- Bilingual preferred.
- Associate’s Degree required.
- Bachelor’s Degree preferred.

Pay Range$72,627.00 – $ (base hourly rate or base annual full‑time salary). Eligible for bonus, commission, or short‑term incentive programs.
