This position, under the supervision of the Manager/Supervisor, involves providing home-based parenting education, developmental assessments, emotional support, and preserving the medical home for at-risk families.
Responsibilities Administrative- Provide referrals and resources based on family needs.
- Assist with monthly billing documentation and data collection.
- Perform clerical duties, including data entry for the home visiting program.
- Follow up on referrals and issues identified.
- Build and maintain relationships within the community.
- Assist in developing community support and education groups.
- Focus on strengthening parent-child relationships and reducing family stress.
- Coordinate play groups.
- Perform initial assessments for potential families.
- Manage referrals and screenings for program admittance.
- Use tools like the Family Stress Checklist for needs assessment.
- Collaborate with the Program Manager/Supervisor on referrals.
- Assist with client satisfaction calls and home observations.
- Create a safe, trusting environment for families.
- Support staff training and orientation.
- Coordinate educational opportunities for staff.
- Report safety incidents promptly and attend safety training.
- Ensure compliance with regulations and maintain valid certifications and licenses.
Education: Associate's Degree in Nursing, Social Work, or related health field required;
Bachelor's preferred.
Certifications & Licenses: BLS (American Heart Association), valid Arizona driver’s license, Fingerprint Clearance Card (application required upon hire, card within 90 days).
Experience: Minimum 3 years in child development, computer proficiency, home visiting experience preferred, bilingual a plus.
Healthcare environment familiarity and computer skills are essential.
