* Job Summary:

** A Treatment Family Home Provider delivers care and support to individuals with emotional, behavioral, or developmental challenges in your home. You will also identified clients in need of added support in the community while they stay in your home where they will receive…. a safe, nurturing environment and work with treatment teams to implement personalized care plans, promoting stability, growth, and independence in not only your home, but their personal home as well.

* Key Responsibilities:

* ** Provide a supportive environment**:
Create a safe, stable, and nurturing home setting.
* ** Implement individualized care plans**:
Work with treatment teams to develop and execute personalized plans.
* ** Deliver emotional support and guidance**:
Offer counseling, mentoring, and coaching to promote emotional growth.
* ** Manage daily living activities**:
Assist with meals, hygiene, and other daily tasks as needed.
* ** Administer medication and treatments**:
Monitor and manage medication regimens and medical treatments.
* ** Maintain communication and collaboration**:
Work with treatment teams, social workers, and other stakeholders.
* ** Ensure safety and crisis management**:
Develop and implement crisis intervention plans and ensure a safe living environment.
* ** Document progress and incidents**:
Maintain accurate records of patient progress, incidents, and concerns.
** Education and/or Experience

Qualifications:

*** A high school diploma or equivalent certificate is required.
* A Bachelor’s Degree in a closely related field is highly preferred.
* Minimum of two (2) years experience of relevant work experience.
*
* Additional Qualifications:

*** A valid driver's license and a reliable vehicle.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Flexibility and adaptability in a fast-paced environment.
