Office Location: Remote within Trillium’s South Central Region of our Catchment Area, which includes the North Carolina Counties:
Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, and Wayne.

What We’re Looking For
Trillium Health Resources currently has multiple career openings for Supervisor Care Managers throughout our catchment area. Supervisor Care Managers play a key advisory role on the Care Management Team. In this role, be responsible for providing guidance, reinforcement, and quality control for the care team and its services across physical health, behavioral health, pharmacy, long-term services and support (LTSS), and unmet health related resource needs.

On a typical day, you might:
- Provide direct supervision and clinical oversight for assigned team of localized, multi-disciplinary team supporting members with behavioral health conditions.
- Ensure staff utilize workflows as required.
- Facilitate learning and development of staff through initial and ongoing training as Tailored Care Management evolves, and novel clinical practices emerge.
- Oversee team members daily work schedules.

Qualifications:
Education/Experience:
Must have a Master’s degree and licensure (LCSW, LMFT, LPA, LCAS, or LCMHC) or be fully licensed as a Registered Nurse (RN) and three (3) years of experience providing care management, case management, or care coordination to the MH/SU/BH population is required.
Preferred Education:
Prefer degree in Human Services Field.
Preferred Experience:
At least one (1) year Supervisory
Experience with two (2) or more staff.
License/Certification:
Must have a valid driver’s license
